Priority Health Asthma Program

Healthy Homes Coalition of West Michigan partners with Priority Health and Corewell Health Visiting Nurses to deliver this program. We know that dealing with asthma can be a drain on your physical and mental well-being, so we’re here to help.

We help people get their asthma under control. This is done by:

• Managing asthma triggers in the home.
• Learning about treatment options to reduce hospital visits.
• Learning how to manage asthma symptoms and attacks.

The program consists of six home visits from one of our Healthy Housing Specialists to identify and reduce asthma triggers in the home. There will also be home visits from a nurse to support the management of asthma symptoms from a medical point of view. All services will be delivered free of cost. You will also receive a free air purifier.
